Which programs need referrals?
For parents or carers looking to self refer, or any provider looking to refer a child or family, please follow the descriptions and complete the referral forms listed.

Developmental Supports (NDIS)
Children 0-8 years with a NDIS plan for early childhood supports can access our programs to promote their development.

Therapy Support (NDIS)
Children over 9 years with a NDIS plan for therapy supports can access our programs to promote their development.

Family Support
Access to PlayAbility Family Support services is free and voluntary with all programs aimed at building the capacity of parents and caregivers.
Indigenous and non-Indigenous families with at least one child 6 years or younger are welcome.

Plan Management (NDIS)
Parents or carers whose children are under 9 years with a NDIS plan early childhood plan with funding for plan management can access this service from us.
We welcome all inquiries about the plan management which begins with a phone call or completion of the Plan Management Request Form.
For parents or carers with a child over 9 years we recommend you seek a more specialised plan management service to handle the different funding options available.
